Why did I receive a statement?

The insurance either applied a patient responsibility to the claim, denied their portion of the bill, or the patient did not have insurance coverage at the time of service. Please contact your insurance carrier directly if you have questions regarding the balance they applied towards the patient’s responsibility when the claim was processed. If you need to submit updated insurance information, please email a copy of the insurance card to frontdesk@acidmd.com.
